SUCCESSFUL IMPLEMENTATION OF A TOTALLY LEADLESS BIVENTRICULAR PACING APPROACH IN TWO PATIENTS

Aims
We report on the implementation of the first totally leadless approach to biventricular pacing utilizing a leadless right ventricular pacing device and a wireless electrode placed in the left ventricle in two patients.
Methods and Results
Two patients who had previously been implanted with a leadless right ventricular pacing system and developed heart failure underwent a procedure to place a wireless receiver in the left ventricle and ultrasound transmitter and battery subcutaneously to facilitate resynchronization of ventricular activation. Procedure times for the placement of the left ventricular receiver were 16 mins and 25 mins, without any complication. QRS width was reduced from 198 msec to 120 msec in one patient and 160 msec to 110 msec in the other. Echo parameters as well as patient’s condition demonstrated the benefit of the biventricular pacing therapy.
Conclusions
This first report of a totally leadless approach for cardiac resynchronization therapy showed safety and efficacy. More studies are required to confirm that approach as an alternative to the standard transvenous resynchronization.
